Hourly Timesheet for Part Time Employees

Completed timesheet due in Human Resources on the 1st and 16th days of each month. See Payroll Schedule for exact dates. Hourly Timesheets are paid on the EXTRA-HOURLY Payroll.

Application for Employment(PDF)

Mail or take completed form to Human Resources.

  • Apply for: Clerical, Secretarial, Service/Maintenance positions, Police Officer and Technical/Paraprofessional positions (Full Time and Part Time)

SOSU Data Information Sheet

Send completed form to Human Resources for processing.

  • Update or change demographic data: Address, Emergency Contact, Education

Amendment to Contract

Complete and submit to Human Resources to start, change, or stop employee contributions to supplemental retirement annuities.

Teachers’ Retirement System of Oklahoma Personal Data Form (PDF)

Send completed form to Human Resources for Processing.

  • Change beneficiary for your Teachers’ Retirement account.
  • Change address with Teachers’ Retirement

Social Security Administration (PDF)

Take completed form to local Social Security office for processing.

  • Apply for an original Social Security card
  • Apply for a duplicate Social Security card
  • Apply for a corrected Social Security card
  • Apply for a change of information on your record

If you change your name, be sure to tell both Social Secirty and Human Resources. This will assure your earnings will be properly reported to SOSU and recorded in the Social Security Administration’s records.

American Fidelity Reimbursement

Send or fax completed forms to American Fidelity for processing.
